Official Contest Rules

This Contest is sponsored by The National Campaign to Prevent Teen and Unplanned Pregnancy, 1776 Massachusetts Avenue, NW, Suite 200, Washington, DC 20036. Contest is void where prohibited. By entering, each entrant accepts and agrees to be bound by these Official Rules.

  1. HOW TO ENTER: No purchase or payment of any kind is necessary to enter or win this Contest. The Contest begins on May 1, 2009 at 6:00 AM PST and ends on May 31, 2009 at 11:59 PM PST (the "Entry Period”). To enter, visit www.StayTeen.org/widget.aspx, complete the list of questions provided, and submit the requested contact information with a valid email address. Limit one entry per person for the duration of the Contest. All entries will be considered made by the authorized account holder of the email address. Duplicate email entries will be disqualified. Persons with multiple email addresses must submit only one entry from one email address. All entrants will be required to provide their email address.

    All entries become the property of Sponsor, and Sponsor reserves the right to use any information submitted by entrants. Sponsor is not responsible for lost, late, illegible, misdirected or mutilated entries, including due to transmission, technical, and/or network failures of any kind, including, without limitation, malfunctioning of any hardware or software (whether originating with sender or Sponsor), human error, or any other error or malfunction. The authorized account holder of the e-mail address submitted at time of entry will be considered the entrant. An "authorized account holder” shall mean the natural person assigned to such e-mail account by the Internet access provider, online service provider, or other organization responsible for assigning e-mail addresses for the domain associated with such e-mail account. A potential winner may be requested to provide proof that he or she is the authorized account holder of the e-mail address associated with a winning entry.

    Entrants agree not to upload, post or transmit any materials which contain any computer viruses, Easter eggs, worms, Trojan Horses or other harmful component or programming routines that are intended to damage, detrimentally interfere with, surreptitiously intercept or expropriate any system, data or personal information. Any attempt to deliberately damage any web site or undermine the operation of the Contest is a violation of criminal and civil laws, and Sponsor reserves the right to seek damages from any person who makes such attempt(s).

    Entry materials that have been tampered with or altered are void. Any questions regarding the number of entries submitted by an individual or the authorized account holder of an e-mail address shall be determined by Sponsor in its sole discretion, and Sponsor reserves the right to disqualify any entries by persons determined to be tampering with or abusing any aspect of the Contest.
  2. ELIGIBILITY: This Contest is open only to legal residents of the United States and the District of Columbia who are between 13 and 19 years of age at the time of entry. Contest is void where prohibited or restricted by law. Employees of Sponsor and its respective parents, subsidiaries, agencies, and affiliates, and their immediate families and persons living in their households are not eligible to participate in this Contest. All federal, state, and local laws apply. Winners will be required to sign and return to Sponsor within fourteen (14) days an affidavit of eligibility and release (except where prohibited) affirming their agreement to all terms, conditions, and restrictions in these Contest rules. If the winner is a minor, then prize will be awarded to winner's parent or legal guardian.
  3. PRIZES: ONE (1) GRAND PRIZE will be awarded. Grand Prize Winner will receive three (3) round-trip airline tickets courtesy of Southwest Airlines, the official airline of The National Campaign to Prevent Teen and Unplanned Pregnancy. The Grand Prize estimated retail value is approximately $1,199.99. Grand Prize must be used by March 31, 2010, and is valid for any destinations serviced by Southwest Airlines. Tickets may be used solely by Winner or by Winner and his or her guests. Grand Prize includes cost of basic airfare and ticket shipping fees only. Grand Prize Winner is responsible for all other costs incidental to the use or enjoyment of Grand Prize, including but not limited to taxes, upgrades, miscellaneous additional airline charges, baggage fees, travel insurance and/or itinerary change fees. TWO (2) SECOND PLACE PRIZES will be awarded. Second Place Prize Winners will receive a 16GB iPod Touch, courtesy of Apple. The Second Prize estimated retail value is approximately $299 each. Second Place Prize includes cost of iPod (including sales tax) and shipping and handling fees only. Second Place Prize Winners are responsible for any and all additional costs associated with the use or enjoyment of prizes.

    Actual retail value of prizes may vary. Grand Prize Winner and Second Place Prize Winners will not receive difference between actual and approximate retail value. Prizes may not be exchanged or redeemed for cash, sold, or traded. No prize transfer, assignment, or substitution by Winners is permitted. Sponsor reserves the right to substitute a prize, in which case a prize of equal or greater value will be awarded. Please allow 6-8 weeks for delivery of prizes.
  4. CONTEST RANDOM DRAWING: Sponsor will select one (1) Grand Prize Winner and two (2) Second Place Prize Winners from among all eligible entries by random drawing on June 3, 2009. Odds of winning depend on the number of eligible entries received. Entrants are required to answer all three quiz questions correctly and provide a valid email address to the Sponsor upon completing the quiz anytime during the Entry Period. To be valid, all entries must be received no later than Sunday, May 31, 2009 11:59 PM (PST). All decisions of Sponsor concerning all matters related to this Contest are final. Winners will be notified via email by June 15, 2009. If any attempted prize notification is not successful within seven (7) days, the prize will be forfeited and Sponsor reserves the right to randomly select an alternate winner from among the remaining eligible entries.
  5. CONDITIONS AND RESTRICTIONS APPLY: Winners must continue to comply with all terms and conditions of these Official Rules and winning is contingent upon fulfilling all requirements. Proof of U.S. or U.S. Territory residency and age are required to claim a prize. Winners are subject to verification by Sponsor, whose decisions are final and binding in all matters related to the Contest. An entrant is not a winner of any prize unless and until entrant's eligibility has been verified and entrant has been notified that verification is complete. In the event that a winner is determined to be ineligible, his or her prize will be forfeited and Sponsor reserves the right to randomly select an alternate winner from among the remaining eligible entries. In the event that a winner is determined to be ineligible or fails to respond to Sponsor within seven (7) days of notification, his or her prize will be forfeited and Sponsor reserves the right to randomly select an alternate winner from among the remaining eligible entries. Sponsor reserves the right to terminate the Contest without awarding any Grand Prize or Second Place Prizes if no eligible winners or alternate winners claim the prizes within the required time.

    Winners will be required to execute and return to Sponsor a liability release and affidavit of eligibility (except where prohibited) within ten (10) days of notice of winning. In the event that a winner fails to execute and return a liability release and affidavit of eligibility (except where prohibited) within ten (10) days of notice of winning, his or her prize may be forfeited and Sponsor may randomly select an alternate winner from among the remaining eligible entries.

    Winners will be solely responsible for any and all local, state, and federal taxes. Sponsor will report any individual winnings over $600 (in cash or fair market value of goods or services) in a one-year period to the Internal Revenue Service. Winners will receive a Form 1099 from Sponsor if Sponsor reports any winnings to the Internal Revenue Service.

    Grand Prize airline tickets must be booked at least thirty (30) days prior to departure. Airfare subject to availability and blackout dates. Travel must be roundtrip. No refund or compensation will be made in the event of the cancellation or delay of any flight. All expenses and incidental travel costs not expressly stated in the package description above, including but not limited to, ground transportation, meals, incidentals, passenger tariffs or duties, surcharges, airport fees, service charges or facility charges, personal charges at lodging, security fees, taxes or other expenses are the responsibility solely of Grand Prize Winner. One traveler must be eighteen (18) years of age or older as of the date of departure and must travel on same itinerary and at the same time as the Winner. Travel companions must execute liability/publicity releases prior to issuance of travel documents. Travel restrictions, conditions and limitations may apply. Sponsor will not replace any lost, mutilated, or stolen tickets, passes, travel vouchers or certificates. By accepting passes, Grand Prize Winner agrees to abide by any terms, conditions and restrictions provided by each pass.

    Winners agree to the use of their names and/or likenesses in further promotional materials and press releases by Sponsor without any additional compensation (except where prohibited). By entering this Contest, winners agree that Sponsor has the right to publicize, by any medium, their names, voices, images, and likenesses, the fact that winners won, and all matters incidental thereto. Winners agree to sign any and all release forms authorizing the use of their names and/or likenesses in the above matter.
  6. LIMITATION ON LIABILITY: Sponsor is not responsible for lost, late, mutilated, illegible, incomplete or misdirected entries. Sponsor is not responsible for technical, hardware or software failures of any kind, lost or unavailable network connections or failed, incomplete, garbled or delayed computer transmissions which may limit an entrant's ability to participate in the contest. Sponsor is not responsible for any typographical, or other error in the printing of the offer, administration of the Contest, or announcement of the prizes and/or all Contest-related materials. By entering this Contest, each entrant forever discharges, releases, and holds harmless Sponsor and each of its parent companies, subsidiaries, affiliates, and each of its respective directors, officers, employees, and agents from any and all liability, claims, losses, damages, causes of action, suits, and demands of any kind arising from or in connection with the Contest, including, without limitation, responsibility for property damages, loss of life, or personal injury resulting from or in connection with participating in the Contest, from or in connection with use or receipt of the Grand Prize or Second Place Prizes, or while preparing for, participating in, and/or traveling to or from any prize-related activity, however caused. Neither the Sponsor nor its respective parents, affiliates, subsidiaries, agencies, divisions nor related companies are responsible for any damages, taxes, or expenses that winners might incur as a result of this Contest or receipt of prize.
  7. SPONSOR'S RESERVATION OF RIGHTS: These Official Rules are subject to modification by Sponsor. In the event of a dispute, all decisions made by Sponsor are final and binding. Sponsor reserves the right, in its sole discretion, to disqualify any person who tampers with or abuses the entry process, or who otherwise acts in violation of these Official Rules. Sponsor reserves the right in its sole discretion to modify, cancel or suspend this Contest should virus, bugs or other causes beyond Sponsor's control corrupt the administration, security, or proper play of this Contest. Sponsor further reserves the right, in its sole discretion, to cancel, terminate, or modify this Contest if, for any reason, the Contest is not capable of completion as planned, including due to force majeure or non-authorized human intervention that compromises or affects the administration, fairness, integrity, or proper conduct of the Contest. If the Contest is cancelled for any reason, winners will be selected from among all entries received prior to cancellation and all prizes will be awarded.
  8. PRIVACY POLICY/DISCLOSURES OF INFORMATION: Sponsor may collect and use information that entrants provide to enter the Contest to actually conduct the Contest. It may further use the personal information to contact the potential Winners. Sponsor may also provide this personal information to prize suppliers so that they may process and deliver prizes to winners. Sponsor DOES NOT SELL any personal information it collects. Personal information includes any information that can be tracked back to a specific individual use, including name, postal address, and telephone number. Sponsor WILL NOT SHARE any personal information it collects WITH ANYONE other than prize suppliers in order to deliver prizes to winners. NO PERSONAL INFORMATION THAT SPONSOR COLLECTS WILL BE GIVEN TO ANYONE WHO MAY CONTACT ENTRANTS FOR ANY PURPOSE NOT RELATED TO THE STAYTEEN.ORG RELATIONSHIP REALITY POP QUIZ.
  9. WINNERS' LIST: To obtain a list of winners, send a stamped, self-addressed envelope to: Relationship Reality Pop Quiz Contest, c/o The National Campaign to Prevent Teen and Unplanned Pregnancy, 1776 Massachusetts Avenue, NW, Suite 200, Washington, DC 20036. Winners' list requests will be fulfilled within four (4) weeks after all prizes have been awarded. Requests for winners' list must be received by June 30, 2009.
